Department of Transportation (DOT) Physical Examination ($95)

 

We perform DOT physical examination on site.  San Diego Lab Testing and its physician affiliates are members of National Registry of Certified Medical Examiners (NRCME).  The certificate would be available immediately after successful completion of the examination.

• On-site/Mobile DOT Physical Service for Companies

 

Companies:

Please call to arrange on-site/mobile services for a set of two or more DOT Physical Exams.

On the day of examination, make sure you take your prescribed medications, specially the blood pressure medications.

 

Please bring the following items to the examination:

• Valid government issued photo ID

• Any appropriate government issued waiver certificate

• Medication list

• ​If any cardiovascular disease, bring a copy of your last examination or stress test.

• If any Obstructive Sleep Apnea (OSA), bring a copy of your last apnea / sleep study test.

 

 

Please follow these simple instructions:

• Fill out the brief health history

• Our Medical Examiner will go over your history and then perform a physical examination in accordance to FMCSA guidelines which will include:

  • review of brief medical history

    • height, weight, blood pressure, pulse rate, range of motion, breathing, eyes, ears

  • Discuss special medical conditions or exemption needs

  • Basic urine test
